As an in home service provider, Kingdom Hearts In-Home LLC cares for patients in their own homes where family support and familiar surroundings help create a level of comfort which enhances their daily lives.
We provide Healthy Children and Youth Program (HCY) services:
This is a comprehensive, primary and preventive health care program for Medicaid eligible children and youth until the age of 21. This allows children to remain in their home as a healthy child or healthy youth. This is not baby sitting. Your child must have a medical condition that warrants approval for HCY services.
Your Plan of Care will ultimately determine the care we provide and MO Medicaid will cover.
Our nurses and health care providers undergo full background screenings. Our health services are available 24 hours a day. Unless otherwise specified, the minimum service is four hours. The health care providers will provide the following services:
Homemaker Chore Services:
Homemaker chore services consist of household cleaning provided
by a trained homemaker. These tasks are necessary for a clean, safe and
healthy environment. Such chores are: laundry, ironing, dusting,
mopping, vacuuming, disinfecting, polishing, medical/essential errands
to grocery shop, pay bills, pharmacy and post office.
Personal Care Services:
Personal care services include: assisting individuals
with activities of daily living such as bathing, dressing, grooming and
assistance with self administered medication. Assistance with toileting
needs for individuals with bowel and bladder incontinence and, medical
related household tasks.

When you just need someone in your home. A nurse will set up the care you need and supervise your care. Personal Care Attendants (PCA) Nurse Aides will provide personal care as well as assistance around the house. These duties include:
We offer specialized services that can be performed on a one-time or recurring basis. These include PCA(s) experienced with: Quadriplegic slide board transfer, Hoyer Lifts and Slings and Bed Baths.
Medication Assistance
How many times have you worried whether or not you took your medicine? Have a Registered Nurse pre-fill your medication box and this is no longer a worry. Once a week, or every two weeks, a nurse will pre-fill your medication boxes and communicate with your doctor.
